The form is required for your initial submission and any subsequent revised manuscript.
Warranty and Transfer of Copyright
A signed copy of this Agreement is required for each manuscript and it must be signed by ALL authors. Please print out this form and send by postal mail only. Fax or online submission will not be accepted. However, if it is not feasible for all authors to sign on the same form, multiple forms can be submitted and signed by individual authors.
